@kara it's annoying to do but you need to call your insurance and figure out why, and what the appeal process is. I'm not as familiar with Washington law so get in touch with a local expert, but your insurance co is definitely legally required to cover some form of estrogen

@claire ah, Multi Speciality Transitions clinic. it's a clinic at the Kaiser campus in Oakland that specializes in helping trans people get appropriate care at Kaiser. it's supposed to be for all of northern California, so they should be able to recommend you a doctor close to you who is trans-competent
