I don't know much about transmedicalism, so I'm very sorry in advance if my question seems naive as hell.

It seems to me it's about deciding for someone if they can call themselves "trans" or not, which is kinda not cool ? (which doesn't justify using a slur for it tho)

Sorry if I'm missing the point, all I know about the subject is from google and It's probably a very one-sided point of view.
