Having looked at the protocol, Mastodon is unlikely to be covertly censoring. Either you know you're blocked, or you aren't blocked.

Not much one can do about State Level Actors... They're often not worth mitigating against when setting up security protocols.
Still, tampering with communications can be detected, if both parties are using pre-shared keys and robust cryptography. Even entities like China's Great Firewall and the NSA have to follow the rules of math.
